We are fortunate to live in such times that silicone breast implants have come full circle. Evolving surgical techniques, anaesthetic drugs and device technology are much safer than a generation ago. Women now have the option of safely changing both the shape and size of their breasts.

When liquid silicone was used as the filler for breast implants, any break in the implant capsule meant that liquid could escape, though it was mostly contained within the fibrous lining around the implant. The cohesive gel that has been in use for more than 10 years has the consistency of “agar” and does not have the tendency to flow like the older liquid gel. The alternative to the cohesive gel is saline or salt water as a filler for the implant, but it has the problem of the salt water falling to the bottom of the implant shell resulting in ridges in the upper part of the implant shell when the implant is in the upright position. One way to minimise the appearance of ridges would be to place the implant behind the muscle of the chest wall.
Those who are not averse to having a cohesive gel implant can have the choice of putting the implant in front or behind the muscle of the chest wall.
The life span of breast implants is variable. Implants can remain in the body for as long as they are trouble free. There is no need to switch implants at a certain time, unlike getting your tyres changed.
Women with adequate breast tissue, such as size “B” breasts, or who have breast-fed and are left with loose skin envelopes or who are into body building or upper body exercises are better having the implants placed in front of the chest wall muscle.
Women who have thin breast skin, or who desire reconstruction after breast cancer surgery are better having the implants placed behind the chest wall muscle, for extra protection of the implant.
Then there are the special cases of those born with breast deformity. The treatment is tailored to the individual, and sometimes the distinction between cosmetic and reconstructive procedures is blurred. However, the aim of the plastic surgeon is to achieve the most aesthetically pleasing result. Sometimes, it requires more than one operation.
Most women have an idea of the size and shape desired, otherwise, magazines and the internet help in the research of the desired look. Once that decision is made, the surgeon can use samples to guide the patient in selecting the correct implant, based on the diameter of the base and the projection.
