What Is Gynaecomastia? What Causes It? And What Are the Treatment Options?

Gynecomastia refers to the development of unusually large mammary glands in men resulting in breast enlargement, or large male breasts. Northern Beaches Plastic Surgeon Dr. Sawjin Tew explores treatments and issues surrounding this often embarrassing condition.
Gynaecomastia describes the increase in ductal tissue and stroma in the male breast. If gynaecomastia occurs in puberty, it is usually reversible, and because of hormonal changes resolves itself by the late teens.
If gynaecomastia persists, one needs to exclude systemic causes such as liver disease, lung cancer, adrenal tumours, pituitary tumours, colon or prostate cancers, thyroid disease, testosterone imbalance, or drug use.
If an underlying cause is not found, gynaecomastia is likely to be idiopathic or arising spontaneously or from an unknown cause which accounts for the majority of cases of gynaecomastia.
What Can I Do About Gynaecomastia?
Once a patient makes the decision to seek treatment, the most suitable procedure depends on whether it is mostly fatty or glandular tissue in the male breast, and whether there is much skin excess.
Where there is minimal skin excess, and if it is mainly fatty tissue, then liposuction would be the most suitable option. Where the gynaecomastia consists of glandular tissue, then excision of the gland, usually via an incision at the inferior edge of the areola may be an option for treatment.
If there is severe skin excess, which will not shrink adequately after the gland is excised, then the skin will have to be excised, leaving a scar on the chest wall.
What Happens If I Don’t Do Anything?
Patients can live with the condition as it is not dangerous. However, some patients are embarrassed when they take their tops off, especially in personal relationships, at the beach or the swimming pool and therefore seek surgical correction.
