The face is the first area of the body where visible signs of aging are first noticed. The skin is exposed to UV exposure, wind and other lifestyle factors such as cigarette smoking. These changes can make the patient look older than they really are.
In considering facial rejuvenation, one can break it down into areas such as:
- Upper face which is the brow and eyelids
- Mid face
- Lower face including jowls and lips
Alternatively, one can consider the components of the face such as:
- Skin
- Wrinkles, both static and dynamic
- Bone structure
The SMAS or superficial musculo-aponeurotic system was first described in 1976 and revolutionized plastic surgeon’s approach to facelifts. It is a layer of muscle and fibrous tissue that runs from the neck into the face and fuses to the cheekbones and scalp. During a SMAS facelift, the skin is separated from the SMAS layer.
The SMAS is then dissected and tightened so the patient gets a full, lifted result that is long lasting. Any excess skin is then tightened and re-draped. A SMAS facelift, otherwise known as a full facelift, is suitable for patients with a saggy neck and jowls.
